The 2016 SMACNA Safety Survey is now available to submit. Please click the following link, where you will be directed to the SMACNA website to login-in and complete the online survey. Upon submitting, a PDF copy of your entry will be sent to the email you provide on the form.
To submit, certain information will be needed that you can find in your company’s 2015 OSHA 300 Log records. In addition, you will also need to know your 2015 experience modification rate (EMR).
ALL ENTRIES MUST BE RECEIVED BY MAY 6, 2016!!!
By completing this survey, you may be eligible to receive a safety award in your man-hour category, which have found to be helpful marketing tools by the companies who have won these awards.
Further information on the 2015 Safety Excellence Awards Program (SSEAP) can be found on the SMACNA safety webpage. Your input is critical to the success of the program. Please be sure to fill in all questions of the survey and submit early.
